Earn rewards for good health

You can earn rewards by doing things that help you stay healthy.

To begin earning rewards, all you have to do is complete the new member health needs assessment or one of the recommended health screenings or tests.

It’s that easy!

Better care, better rewards

Members may earn:

• $20 completion of all six well-child visits (birth to 15 months).
• $15 annual well visits (ages 2 – 20).
• $20 annually for adult well visits (ages 21 and older).
• $5 for annual flu shot.
• $25 (per household) for opting in to the mobile texting campaign.*
  *Members must not opt out of receiving mobile app notifications to qualify for the benefits.

Women may earn:

• $20 for completing a postpartum visit within 7 – 84 days of delivery.
• $10 for cervical cancer screening (women ages 21 – 64).
• $25 for a prenatal visit in first 12 weeks of pregnancy.

Members with diabetes may earn:

• $10 annually for completing all three: hemoglobin an A1c (HbA1c) testing, dilated eye exam, and kidney screening.

In 2026, we are offering more rewards for healthy actions:

• $50 for completing your health needs assessment within 60 days of enrollment online
• $30 for completing your health needs assessment within 90 days of enrollment by phone or paper
• $120 for six well-baby visits ($20 per visit up to $120, birth to 15 months)
• $25 for yearly well-child visits
• $50 for yearly adult well visits
• $50 for colorectal cancer screening
• $50 for cervical cancer screening
• $25 for breast cancer screening
• $25 for diabetic eye exam (ages 18 to 75)
• $30 for diabetes screening
• $35 for STI screening
• $25 for needed vaccines by age 2
• $50 for needed vaccines by age 13
• $15 every three months for high blood pressure medication fills (up to $60 per year)
• $25 for completing a follow-up visit within 30 days of an ER visit for substance use and/or mental illness
• $25 for signing up for our mobile app or opting into our mobile messaging platform (one per household, one-time benefit)

Your CARE Card

Activation:

To activate your card, please call 1-888-682-2400 and follow the instructions. You can also activate your card by visiting my benefits center. You may need your health plan ID card with you to complete activation.

Visit a participating retailer:

You may use your card at any participating retailer. You do not need a prescription and do not need to go to the pharmacy window. Remember to take your card with you — you will need the card to pay for your eligible items.

Pay cashier:

Gather your eligible items and proceed to the store check-out lanes. When you check out, swipe the card for payment. Eligible items up to your available card balance can be paid for with your card.

Any remaining balance will carry over until your next purchase, and new rewards will be loaded as you earn them.

*You cannot use your rewards to purchase alcohol, tobacco, or firearms. Rewards expire 12 months after your most recent reward or upon member disenrollment. Your rewards may not be converted to cash.

Eligible CARE Card program rewards are subject to change. AmeriHealth Caritas Louisiana will notify you before the change happens.

Once your doctor notifies us that you have completed a healthy activity we will add rewards to your card. Members may not be eligible to earn all of the rewards listed. For questions, call Member Services at 1-888-756-0004 (TTY 1-866-428-7588) 24 hours a day, seven days a week.
