Medication Adherence
One of AmeriHealth Caritas Louisiana’s goals is an effort to increase our members' medication adherence with regard to asthma.
According to a December 2012 report published in the Annals of Internal Medicine, "non-adherence has been estimated to cost the U.S. health care system between $100 billion and $289 billion annually," and to be the cause for at least 10 percent of all hospitalizations.
We have tools and programs to help you identify members who are struggling to adhere to their care plan. Learn more about medication adherence (PDF).
Healthy Hoops®
Healthy Hoops is an asthma education program that AmeriHealth Caritas Louisiana and its Healthy Hoops Coalition partners use to teach children and their families how to manage asthma through appropriate medication usage, proper nutrition, and monitored exercise.
This innovative program includes hands-on asthma education plus basketball coaching sessions for the children from local coaches, and entertainment for the whole family. The asthma education includes health assessments, spirometry screenings (to test lung capacity), asthma, cardiovascular, and nutrition education sessions, and parent education sessions.
Care Management Asthma Control Program
You may refer our members with asthma to the AmeriHealth Caritas Louisiana Care Management department to sign up for an Asthma Navigation program. Please call 1-888-756-0004. We want to help your patients live a healthy life. Encourage them to call a Care Manager today.
