In an effort to help you through this process, we have outlined the information and key resources needed for physicians/practitioners and facilities below. Please be sure to double check the information provided, especially federal employee identification numbers, service and billing addresses and phone numbers, to ensure they are current. This information will be listed in the member directories to guide them to your practice and for us to set up your claims payment.

Need a provider agreement/contract? Please contact us at 877-588-2248 or via email at lacarenetwork@lacarelouisiana.com. One of our local representatives will contact you to answer any questions and send you the agreement type that is best suited for your practice/facility. Examples include: Primary care, FQHC/RHC, specialist, ancillary, hospital.
