AmeriHealth Caritas Louisiana has a maternity case management program called Bright Start. The program is designed to improve birth outcomes and reduce pregnancy-related complications through early prenatal education and intervention.
Bright Start combines scheduled written and telephonic outreach that provides point-of-contact notification of health needs to members. Bright Start uses provider and community programs, partnerships, and creative outreach strategies to facilitate member access to required services.

Notification of Pregnancy Form
The Notification of Pregnancy (NOP) form (PDF) gives our Bright Start maternity care management team the best opportunity to appropriately assist with managing our maternity members. Providers should complete the NOP form as early as possible in each expectant member's pregnancy. Fax the form to 1-888-877-5925.
Home injections for 17P compound
You may use the Optum referral form (PDF) to request to have 17P compound injections performed in a member's home. Please choose "Other" in the "Recurrent preterm birth management program" section and fill in your orders for compound injections to be provided in the home.
